Add the preferred pharmacy to all faxing options

Dr. Berthelot would appreciate (and believes other clinics would too) to have the option of the preferred pharmacies available in all the faxing options all over MYLE to reduce the need of enterring manually the fax numbers.

He would also appreciate an option to fax it directly to their own clinic, in the same spirit as the preferred pharmacy option but to add a third so: preferred pharmacy > clinic itself > custom option.
