Ability to attach a note to documents that are shared to the portal.

The ability to attach a note would only need to apply to documents that have been shared to the portal, but that have not yet been consulted by the patient whose portal account they were shared to. This would let the secretaries know if and when a patient has been contacted regarding the shared document.
