Rename to Lifestyle or Social History

When an Encounter Note enabled/active, the Social Habits is condensed into multiple items displaying in one+ row. Instead of the Active Problems displaying one item per row. The physician would like to see this similar to the Active Problems

1. in addition to display the "note" of the Social Habits within

2. see attached (both have 4 new entries) active problems, each new entry is on a new row. whereas, the social habits all 4 entries are within the same row making its harder to read
