Demographic data in duplicate forms are not updated after change in the patient banner

– Affected user(s): Forms users.

– Task affected: when a form is duplicated or continued, and in the meantime the user's demographic has been modified (e.g. name, Nam, address, etc.), the user's demographic info entered in the duplicated or continued form is the old one, not the current one in the file.

– Impact : The user has t re-enter all the form data. The client would like to ensure that the user's address corresponds to the one in the banner or in the Demographics tab of the user's file when duplicating or continuing a form.
