Update sections to state "Primary Care Provider"

In BC Nurses and Physicians can be the "Primary Care Provider".

The nurses would like this updated in the following sections to reflect either type of provider. In addition, each section states a different type of provider, nurse, specialist, etc.

• admin > user > practice > title / practitioner number

• patient profile > clinicians
