Create a customizable sticker in the patients file

Users are often unsatisfied with the sticker options offered in the file. Some stickers have too much info on them (ex: C1, C2, C3) and others have not enough info.

Users would like the option to customize a sticker. Ex, when they would select that sticker option, they could select what they want to add to it (ex: name, DOB, health card number, family doctor, etc.) and then print it.
