Adding another field in the document details

Add a field in Exam queries, and the same field in documents, with a drop-down list (as in Class) where you can choose the type of document (x-ray, for example).

The purpose is that when the Dr is creating an exam request and selects type X from the drop-down list, MYLE suggests the documents to be attached that are of type X.
