Be able to see the phone number and fax number of the recipient in the Recipient section of Consultation Request

Update request for the Consultation Request section of medical notes:

Users would like to see the phone number and fax number of the recipient in the Recipient section so they can copy/paste the fax number in the Fax Consultation Request after clicking on the "Fax" option.

At this moment, they only see the phone number.
