Portal: ability to add a kid when self-creating a portal account

When a patient create a portal account (via a clinic website or mass email), there is no way to add a kid so the parent that needs an appointment for his kid will have to book the appointment under himself and bring the kid to the appointment.

Tis creates confusion as some doctors are not seeing kids or are not expecting to see a kid when it's an adult that took the appointment.
