Clinic Site adress/# independant from portal for privacy esp Telemed services

Client wishes to edit what the patient sees when taking appointment online with her (Full Telemed clinic). Right now what shows in the portal, is whatever is in Admin>Site, as is the case for forms, prescription, consultation req etc. The option to break this link or at least have a selectable choice to unlink if necessary (through the service itself perhaps?). See attached for current patient's view, in this scenario the client wishes no personal info to be show(nor address nor phone number).
