FAX DIRECTORY: Add option of PHONE NUMBER to the creation / modification of Professional type of entries.

Currently: We can add a phone number when we link an establishment to the Professional's entry in the directory, but we do not have a direct option to add only the phone number.

Improvement idea: Add a section specific to the phone number in the Professional creation / modification, similar to how it is presented in the Establishment section.

Impact: Reduce the need to attach an establishment
