Attach documents to a clinicien should be mandatory in MYLE

Any documents in MYLE that is related to a patient file. Mostly, if it's a result or an important prescription, it should be mandatory to add a clinician attached to it.

This will avoid human mistakes of missing an important follow-up with the patient.
