CALENDAR: Add an option to search a patient by referring physician

Users would like an option like a checkbox in the CALENDAR tab that will allow them to search a patient or patients by referring physician.

Sometimes they have to go through the whole list to find the patient because they only have little information including the referring physician's name and/or permit number.
