FORMS: Add "ÉVALUATION DE L’AUTONOMIE MULTICLIENTÈLE" form in the same format as the Obstetrical Forms

User requested to have the following form in the same format as the Obstetrical Forms

– Affected user(s): Users (Physicians) who use that form

– Task affected: Filling that form as fast and efficiently as possible

– Impact : Waste of time every time they need to continue that form from a previous note.

– Current state : They need to search the form in the forms list, fill it and add the star so they will see it in the next note. They'll need to open the form again to see was inside and go back and forward between the note and the form to fill it. They don't have this issue when they use Obstetrical Forms and "ÉVALUATION DE L’AUTONOMIE MULTICLIENTÈLE" is a form they use very often.
