Add the ability to configure recurring forms in notes by user not just by patient

– Current state : Presently in MYLE, any user can set a form to recur in every note created in a particular file, and it will appear in every note created by all subsequent users.

To clarify the clients request:

Presently in MYLE, any user can set a form to recur in every note created in a particular file, and it will appear in every note created by all subsequent users.
What they would like to see, is that when a user sets a form to recur in subsequent notes, it will only auto-populate for that user.

Doing this would allow all users to select different forms that will generate in their notes without the forms populating in the notes of other intervenors.
