Add option to identify 'in-person', 'by phone' or 'virtual' for consultation type.

1. make it such that a phone call can be initiated from the patient's file (via teleconsult) and be converted to a videoconference if needed
2. adjust the service categories to match the hub's service categories, which also includes home visits
