Add customization options to be able to select which sections are printed when printing notes from a patient file

To clarify:

At the moment, in MYLE, you cannot choose which sections of the note that will print. By default all of the sections that have information entered will be printed.

Client wants to be able to choose for themselves which sections are printed.
