PORTAL: Ability for MOA/Providers to approve the appointments

Currently: Patient books an appointment, it is confirmed by the system and the clinic can delete the appointment if necessary.

Improvement idea: To allow patients to request an appointment and for the clinic to approve it instead of it being automatically confirmed.
