Allow clinics to modify the text for the confirmation and notification messages.

-ability for users to create/modify templates for the notification/confirmation text based on the type of appointment
-ability for users to create/modify templates for the notification/confirmation text based on the physician for the appointment
Similarly, clinics often add descriptions and disclaimer to services but these only show in the pt portal when booking an appointment and not in the appointment notification.
By allowing clinics to create their own appointment notifications, they could choose to add these info.
