[NOTIFICATIONS] Questionnaire - Have the questionnaire notification bear the name of the appointment site instead of the whole clinic

• Reported by France Lauzon, from gmfdessommets, but impacts all multisite environments.

• This mostly impacts administrative personnel and patients directly, but also affects providers by extent.

• This would only affect multisite environments that use questionnaires.

Task affected:

• When an appointment with a questionnaire sends out its notifications, it lists the questionnaire under the environment's name, and then the appointment notification indicates the site (or location) name.

Impact:

• According to the user, patients stop reading or get confused by seeing the name of the environment at the top of the notifications, and make their way to the wrong clinic site for their appointment. They then have to be redirected to the proper clinic site, making them late for their appointment.

• This, in turn, wastes time for the administrative personnel at the erroneous site, and for the clinical personnel at the actual appointment site where the patient arrives late.
