[COMMUNICATIONS] Reminders - Add a column to hold the patient's birth date

• Would affect all clerical personnel scheduling appointments from reminders, more specifically in pediatric clinics.

Task affected:

• Scheduling appointments for patients that were entries in the reminder section of the communications tab.

Impact:

• In a pediatric clinic, all reminder appointments would benefit from this improvement.

• Without an at a glance view of the patient's date of birth/age, it is not easy to know in which type of services (1 year old and under, 5 year old and under, etc) the patient should be scheduled.

Current state:

• Currently, the user has to access the patient's chart to note their age, and then handle the reminder from the communications box.
