Vulnerability code - add en end date

– Affected user(s): Treating staff doing the enrolment

– Task affected: When adding the vulnerability code

– Impact : They want to have the option to add an end date to when adding a vulnerability code

– Current state They need to go to the RAMQ site to modify the vulnerability code manually to end it
