Allow 'Share in future notes' function to be user specific

Currently, selecting a form to be shared in future notes displays the form in all future notes within a patient chart.

In large establishments such as CCSMTL and CSSSJM, multiple types of professionals can be involved in a patient chart (ex. a nurse for wound care, and a social worker for interventions).

If a nurse has selected multiple forms to be shared in future notes, these forms are displayed in the social worker's notes, and are also pushed to OACIS unless the social worker either a) manually deletes each form or b) selects to hide each form from future notes. In both cases, each user has to manually reselect desired forms individually.

Suggested improvement : allow 'share in future notes' function to be user specific. The user who has selected 'share in future notes' will see that form displayed in future notes that will be opened by this same user, while other users will not see that form displayed in their notes.
