Consultation Request View

• When accessing referral forms it can be difficult to access needed information. When

first looking at forms under “FOLLOWUPS”, while the reason and description is listed, the

doctor providing the consult report is not included. The omittance of the physician’s name

make it more difficult to find necessary documents, as they must be individually opened to find

the physician’s name.

• once a consultation report in selected, there is no way to access the

address of the consulting physician other than reading the report. Our staff would like to see a

tab for address under the consultation request window. This way, we can easily provide the

physician’s address to patients.
