Chart Access: Add ability to set more permissions for lists (as an example)

- List modules: we should be able to grant access to a list (versus modify)
- Access to chart per title and level of information shown

DEV : Denying access to some patient charts, or wanting to remove access to non-medical employees? How is this different than ID #78?). We're looking at bunching IDs #77, 78 and 113 and giving them all evaluations between X-LARGE and Ꝏ
