GAP report to show one line per time slot

Affected users: users managing the appointments.

Task affected: The report generated by MYLE (Export Time Slots) may contain more than one line per an open time slots. This makes the comparison to the reports sent by Petal MD more complicated. The client would like to have the option to make the report generated by MYLE display only one line per availability.

Impact: Eport Time slots reports

Current state: The client should go through each time slots with more than one line to check the reason why.
