Add the possibility for patients to add their home address and pharmacy informations when creating their portal account

Task affected: Updating patient files

Impact: It would save a lot of time when creating a new chart to link a portal account to if those demographics were already added when the patient created their account.

Current state: MYLE users needs to manually add this information each time, since there's no field to add this information when the patient creates their portal account
