Medical Summary default information <--Add Pharmacie and Family Doctor in <--

Client suggests to Add family doctor and Pharmacy preference in the Medical Summary form , currently only patient information is indicated in the banner and no info of the actual Doctor who takes care officially of the patient nor the main Pharmacy of the patient aswell mentioned on the summary.
