Update the date when the 'Duplicate' feature for froms is used

Affected user(s): Doctors, Nurses, notes creators

Task affected: Duplicating forms

Impact: It would save a lot of times if every field indicating the date the form was added would automatically update to today's date

Current state: The old date is duplicated, and users need to correct this field on many pages, depending on the form duplicated.
