History of appointments - add more filters

Clients who want to print the list of upcoming appointments to give to patients would like to :

• Be able to hide cancelled appointments, so as not to confuse patients

• Have appointments be listed in the reverse order (closest to furthest, rather than the current view of furthest to closest), as the patient looks at upcoming appointments from top to bottom.
