CareWay - Add ability to personalize the format of notes (font, size of text, etc)

Users would like to be able to personalize the format of notes (font, size of text, etc)

Users would also like to be able to add the heading/logo of the clinic when printing the note. Useful in cases where the clinic/hospital does not use an EMR.
