Pharmacy - ability to add a note in the file

Requested by the client cmlus.

HCP calling pharmacies need to listen to a menu in order to know which extension to choose.

They would like to add a note in the pharmacy information to know which extension to choose quickly.

Current workaround is to add the information in the Insurances section
