[PATIENT PORTAL] Synchronisation - Implement a confirmation pop up when linking a patient portal to a patient chart

Affected users: Medical Office Administrators (MOAs) and administrative staff responsible for managing portal registrations.

Task affected: This occurs in the patient chart synchronization workflow. Specifically, when an MOA links an existing portal account (created by a patient) to a physical patient file within MYLE.

Impact: High risk of data breach or medical error. Currently, the linking process is a "one-click" action without a validation step. If an MOA accidentally links a portal account to the wrong patient chart, the patient could gain unauthorized access to someone else's sensitive medical information, results, etc.

Current state: The system currently allows a direct link with a single click.
