• Impact: High daily frequency across most patient encounters. Currently, the workflow is interrupted, leading to significant productivity loss and a higher risk of data entry omission because the clinician must navigate away from their current task.

• Current state: To add a new entry (e.g., a new diagnosis or a vaccine), the physician must close or leave their current clinical note or result view, navigate to the specific section in the patient file, add the item, and then return to their previous work. This requires multiple clicks and causes a loss of visual context.
