Portal - adjust name of fields to fill when a patient creates a portal account

Currently, when a patient creates a portal account, they have these fields to fill out.
Patients have a tendency to input their Family Name as their First Name and vice versa. This creates issues later when trying to link portal accounts to patient files, and also creates issues with the RAMQ.

Suggestion :
In French, name the field 'Nom de famille', to make it more clear compared to 'Nom'.

Other suggestion from the client : for the Preferred Name field, add a note mentionning that this field is optional.
