Add an option to set appointment status to "In consultation" instead of "In triage" for unassociated clinicians

Affected users: Residents and Administrative staff.

Task affected: Creating or modifying a medical note by a clinician who is not officially associated with the appointment (e.g., a resident).

Impact: High frequency in teaching clinics. The current hardcoded rule automatically switches the appointment status from "In waiting room" to "In triage" when a resident opens the note. This creates significant confusion for the administrative team managing the schedule, as patients may be left waiting because their status incorrectly reads "In triage".

Current state: Administrative staff must manually correct the appointment status back to "In consultation" every time a resident starts a note. Currently, there is no configuration or toggle available in MYLE to disable or customize this automatic status transition.
