Add Travel and Public Health Monitored Infectious Disease Screening Questions to the Standard Intake Questionnaires

Affected users: Medical Office Assistants (MOAs) and clinic administrative staff responsible for patient intake and triage.

Task affected: Patient check-in and intake triage workflow within MYLE when completing the standard integrated COVID and Measles screening questionnaire.

Impact: High frequency (used at every patient visit). Without these questions integrated into the standard questionnaire, there is an increased risk of failing to identify potentially contagious patients (e.g., Measles, Mpox, Ebola, Whooping cough) upon arrival, posing an infection control and public health risk to clinic staff and other patients.

Current state: Unknown / manual verbal screening. Staff either ask these questions verbally outside the system or must document responses in manual clinical notes, which creates inconsistency in patient screening records.

Specific questions the client wants:

Have you had, or been in contact with someone who has, a disease monitored by Public Health, such as measles, whooping cough, mpox, tuberculosis, Ebola, or another monitored disease, within the past 3 weeks?

Have you travelled to Saudi Arabia, Nigeria, Uganda, the Democratic Republic of the Congo (DRC), or South Sudan within the past 21 days?
