Patient Portal Indicator for patients with a representative

When a patient has a representative (or Guardian) linked to this child's patient portal, the color of the icon is 'orange' which indicates pending.

We should change the color of the child's patient portal to 'green' when a parent links the child's patient portal to their own.

This is to help MOAs better identify the status of the portal account.
