Have a print button so the MOA can right from the Enrolment tab print the form for the patient.
This is a legal Form : Formulaire de consentement au partage et a la communication des renseignements sur l'usager.
From the RAMQ website, when inscribing a patient with a Family Doctor, the legal form is now 3 pages.
The inscription itself, followed by these 2 pages.

Nicholas said: The real problem is that this task is more precisely done by clerical staff, who do not have access to Clinical Notes to access the "Forms" area. As such, we give them a pdf form they have to fill outside of MYLE without any dynamic zones (or patient pre-filled in information). It’s a pity because it is not as stream-lined and convenient.
