Add the possibility to modify entries in Relationships tab

When a relationship is entered in a patient file, it's impossible to modify the datas.

For example, to add a people as a Representative, Mother or Father you have to know and to do everything in a row. Usually, MOA, add the patient, but they have to come back later to add the RAMQ number for example. Actually, it's impossible to act like that.

Other example : If the relationship is there (DT), but the RAMQ number is not entered, you just have to remove an type it again EVERYTHING. It's very difficult to explain.
