Deactivating a folder - Adding reasons

This client has very particular and uncommon processes. When a child is referred to their clinics a record is created. The file must be evaluated. The patient may not be accepted. This refused file should not appear in the statistics. We want to deactivate the patient. The client would like to add the following status when deactivating the file:

The child no longer needs CHP services;

Child is referred to the SSCP;

Moving;

The child has reached the age of majority (18 years);

Death.
