and custom descriptions as well (to save them)

This was also requested by nhc, to have more class option available:
Results - Pathology
Summary - Emergency room
Clinical note - Emergency room

many clinics are asking for this one, I got cmcc that is quite insistent on this one.

Client would like to be able to add personalized categories class such as :

• Facture (invoice)

• Protocole (Protocol)

• Photo (Photo)

• Note opératoire (Operating note)

As well, private clinics don't have the same needs as a RAMQ clinic.
