Add time of visit and patient's birth date + age in the Portfolio Printout

For the NB and facturation purpose, the MD need a print out with the birthdate, the age of the patient, the date and time of the visit, and ideally the service (medecine sur place ou telephonique).

We all know that homologation process in NB is on the way, but this really could be a quick win.

Possible to add that in the printout ?
