Automatically add the reason "Renewal" in notes created by renewing prescriptions from the RX RENEWALS tab

RENEMAL: When a note is automatically created when doctors renew a prescription; it would be interesting if, in the Reason section of the note, the mention of "Renewal" appeared straightaway.

Ability for users to add a Description column the the Rx Renewals module

This would permit users to see a brief description of the renewals. Users have mentioned that they do not use the source column and they would prefer to have a Description column instead.
