Add feature to print information transfer form directly from the patient's file

– Affected user(s): Users with a clerical account.

– Task affected: When a patient shows up to the medical secretaries and need to file out a form for information transfer.

– Impact : Ms. Tessier has to deal with this task a lot. She would like for the form to be automatically prefilled when we generate it from the patient's file, so that the date and identity of the patient is already populated. See Ms. Tessier example Form. It would save them a lot of time.

– Current state : Since clerical accounts aren't able to create a note and add those forms, they can't have this form prefilled with all the information. They need to print that form all empty.
