[RAMQ] Consent forms - Possibility for the consent form to take into account the patient's legal guardian when autofilling names

• This impacts all staff from pediatric clinics.

Task affected:

• This impact is felt every time a member of the clinic personnel needs to produce an enrollment consent form.

Impact:

• This impact is felt every time a new pediatric patient (or patient under 14 years old) needs to be enrolled to a provider.

• Overall, the office assistants (or whoever is creating the consent form) has to head into pages 2 and 3 and remove the minor patient's name to replace it by their legal guardian's name.

Current state:

• There is currently no workaround for this; the user has to generate the form, then manually empty the fields and fill them back up by typing the name of the legal guardian.
