Current state:
Users only see the PHN next to the patient's name. Sometimes the PHN isn't on the document so they have to go by the date of birth on the document
Requested by Vitalité. They have many patients with the same name. It would be helpful to have more information about the patient (date of birth, attending physician, etc.) in order to identify the correct patient more quickly.
