[Patient Portal] Email correction - Rework email change interface to make it less confusing for patient and user alike

Affected users: Primarily MOAs and administrative staff, though it affects any user managing patient portal access. This impact is universal for all clinics using the Patient Portal.

Task affected: Within the patient chart, specifically in the Portal tab when clicking the "Change" button to update a patient's portal username/email.

Impact: High frequency depending on patient volume. The current interface displays the old email address during the transition, leading staff to give incorrect instructions to patients. This results in patient frustration, failed troubleshooting, and unnecessary follow-up calls when the patient looks for a link in the wrong inbox.

Current state: Users must ignore the displayed email address and manually remember that the link was actually sent to the new address. There is currently no visual reminder that changing the contact email in the profile does not automatically update the portal login.
