Billed TO vs. Billed FOR

Currently, the private billing module generates invoices which are not accepted by some organizations (Dynacare, for e.g.).

Instead of saying "Billed FOR X patient", it says "Billed TO". Clinics would like to have both options, and to select the person/organization.

Currently, a lot of clients print it and modify it because Dynacare sends them back.
