Update "Consultation Request" to toggle an option to state "Consultation note" for specialists to use instead of consultation request.

"Save Settings" within each section

Unify the "Save Settings" checkbox to display in the same place for each section (save is an option). SUGGESTION to move the checkbox closer to "OK" on the bottom right of the window.
